Please bring the completed and signed forms to the first visit, along with patient ID, insurance card(s) and a form of payment for any copayments due at the visit. We accept cash, check, and all major credit cards.

Our HIPPA and financial policies can be accessed by opening the same new patient forms file and scrolling to the corresponding form.
